1. Covid-19 Policy, explaining the new protocols we will be observing.
2. Covid-19 pre-treatment questionnaire to fill in, sign and return. This includes questions about your health regarding Covid-19, such as have you been tested or seen anyone who has it?
3. Covid-19 Disclaimer to sign and return.

These are to ensure yours and my safety.
